From the summary: "Hope is a defining feature of a patient’s use of artificial reproductive technologies, and with it come vulnerabilities. Many patients risk attaching false hopes to “add-on” interventions to in vitro fertilization, which are heavily marketed despite a lack of evidence of their efficacy. To avoid exploiting patients’ hopes, ART providers should make add-ons available only through research protocols rather than by selling them. The health of our planet is critical to all life, including humans. Most human activities, or at least the ways we currently do these activities, are detrimental to planetary health. If the global healthcare system was a country, it would be the 5th largest emmitter of greenhouse gases. In addition there are single-use plastics, disposable products (e.g. masks), power useage and discarded medications and chemicals. We must transform our healthcare system to be sustainable. A few weeks ago, Nicola Rivers and I challenged this year's students in the Master of Clinical Embryology degree in the at Monash University to critically review our labs in key areas of sustainability. The students took on the task with a real sense of purpose and realised they have influence, their voices matter and they can make a difference with their choices. These students will become Clinical Embryologists working in human assisted reproductive technology (ART) and IVF around the world. Another interesting article by David Sable on the use of AI for embryo selection in the IVF clinic. This approach promises to improve embryo selection for transfer by correlating embryo image analysis with developmental competence. It might prove useful in the longterm, but makes several assumptions that are hard to trust. For example, how could morphokinetic analysis provide any useful information on the biggest predictor of embryo health: chromosome status (euploid vs. aneuploid)? I would be surprised if even AI could ever pick up karyotypic anomalies that arise in embryos using light microscopy timelapse imaging. 🤖 🧬 Another major point that morphokinetic image analysis will never address the often low numbers of embryos available to even make a selection. This is especially true for older patients. Overall, innovation in ReproTech should focus on ways of treating infertility rather than predicting tea leaves. 🫖 🍃
