The "Doula -Dash Syndrome" as articulated by @held_massage_therapy is a compassionate and important framing about why and how doulas have need for more funding sources for steady income. 🌍 We asked students from our online Master of Family Medicine programme about global health and the biggest challenges facing family medicine practitioners.
Tania Gahama Ineza from Burundi told us the role of primary care in global health was an important issue: "In many parts of the world, it doesn't have a huge part in global health, and particularly in my setting, primary care is not well-developed. We need to advocate for more intervention in primary care."
For Arthur Liu from Taiwan, the fragmentation of primary care is an issue: "A general practitioner can bridge the gap of medical care."
And for Shavon Alleyne from Ghana, the ability to provide patient-centred care is an important global health challenge: "It's very important that health care professionals understand the importance of having a patient-centred approach towards providing health care.
‘ABCD of Private Practice!’

Hmm…here comes some more gyan!’ I thought, when I first heard a senior say this acronym during my fellowship days...
After 2 decades of practice – with my fair share of professional struggles, starting with consulting jobs at various clinics & later founding my own eye hospital - I admit that the acronym ABCD has not only stuck, but also served me really well all these years. Here’s what it means:
A for Availability: Nothings bugs patients more than not finding you at the committed time. In my consulting days, the only times I got ‘not so good’ remarks from patients was when I was late to an appointment. I found that people are patient if they're told how much they must wait. Communication & sticking to your commitment is the no.1 value in practice.
B for Behaviour: ‘Doctor Saab, aadhi bimari toh aapse baat karke duur ho jati hain’. I’m sure you’ve heard this before.
‘Greet your patient, introduce yourself & then start examining’- this was drilled into our brains during fellowship. When I returned to my hometown & started practicing this, it felt awkward at first. But a few months in, a patient said, 'I will only get operated at your centre, because nobody has ever called me __Jee!' Yes, at our centre, no one called the patients by name. You add Mr before the name or Jee after; I was super finicky about this. My staff started using both, initially out of fear, but soon realized what they got in return – respect.
C for Compassion: I was contemplating to do 'Keratoplasty' on a child. But chances of success were bleak. I conveyed this to the parents. They requested me to speak to their referring doctor, who said, ‘Dr Saab, you go ahead & do what you’d do for your own son.’ For the first time, I thought – ‘what would I do with my son?’
At first, I refused to entertain the idea of my son going through something so severe, but later, calmed my nerves & thought about it. I told them pros & cons of the procedure, but this time, also explained 'why' I would do it. I was experiencing Compassion..LIVE!
Now whenever I am in a fix, I place my family members’ in the patient’s shoes & get my answer. And guess what, irrespective of their education/ IQ/ socioeconomic status, all patients can perceive if you truly care.
D for Degree: It comes last, but it comes. There are doctors not technically great but doing very well because of A-B-C. And there are ones decorated with many degrees, but struggling to attract patients…because of paucity of A-B-C.
Those who do exceptionally well are those who have D too - frequently upgrading yourself!
Adding new technology/skill sets/ facility etc is a ticket to evolution!
