JOB TITLE: Intake Specialist

LOCATION: Cheboygan, MI, 49721

REPORTS TO: Director of Access, Quality and Outcomes

STATUS: Full-Time, Non-Union, Exempt

HYBRID ELIGIBLE: This is a community-based position

STARTING WAGE RANGE: $58,718 - $61,069 per year

QUALIFICATIONS:

Education: Master’s Social Worker, or a Limited Licensed Professional Counselor or Psychologist with appropriate State of Michigan Licensure and/or certifications. Salary and range will commensurate with education and experience.

Experience: Minimum one year of experience required. Must be a Qualified Mental Health Professional (QMHP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND

is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field.

Other: Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical and mental health, chronic health conditions, and behavioral health medications. Lived experiences with behavioral health issues are valued.

SUMMARY OF RESPONSIBILITIES:

The Intake Specialist conducts a comprehensive psychosocial assessment for individuals seeking mental health services, including co-occurring SUD services, to determine eligibility and treatment recommendations. The Intake Specialist collaborates with the individual to obtain the appropriate level of care either within the agency or in referral to another provider. The individual in this position performs risk assessments and mental status evaluations and assists with psychiatric hospitalization, if indicated. Other responsibilities include serving on the emergency services rotation team, providing after-hours crisis intervention, pre-screenings for psychiatric hospitalization, and treatment recommendations under the direction of the Director of Access, Quality and Outcomes.

ESSENTIAL JOB FUNCTIONS:

  • Provides timely, professional intake assessments for persons with severe and persistent Mental Illness, Intellectual or Developmental Disabilities, Autism, and Children with severe emotional disturbance. Provides SUD assessments when indicated.
  • Works in collaboration with the Access Center to ensure individuals receive timely access to services.
  • Collaborates with clients in assessing their needs, creating crisis action plans, and obtaining the appropriate level of care.
  • Performs risk assessment and mental status examinations and makes recommendations regarding psychiatric hospitalization (if indicated) in collaboration with emergency room personal, law enforcement, and court.
  • Provides crisis assessment and disposition to walk-ins, as needed.
  • Maintains knowledge of mental health symptomatology, interventions, and treatment strategies. Provides provisional diagnoses.
  • Maintains a referral resource file for persons in need of services not meeting qualified severity indexes. Maintains referral resource file to assist in access to housing, medical care, food, heat, or other life needs.
  • Utilizes the consultation services of NCCMH psychiatrists and supervisor.
  • Provides consultation to other community agencies and professionals (police, courts, etc.) as needed.
  • Maintains good interagency relations with community resources and services.
  • Provides complete and timely documentation for the electronic health record.
  • Participates in weekend rotation of emergency services.
  • Maintains working knowledge of MDHHS Contract Technical Requirements and Medicaid Provider Manual relevant to job duties.
  • Performs other related duties as assigned by supervisor.


PHYSICAL REQUIREMENTS:

Ability to walk, bend, stand, sit, lift to 50 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in CPI techniques which will be a training class provided to the job incumbent. OTHER REQUIREMENTS:

  • Must be current in Adult CPR and First Aid; provided by NCCMH.
  • Complete initial training in Non-Violent Crisis Intervention (CPI); provided by NCCMH.
  • Have own phone for communication and reliable access to messages.
  • Have reliable transportation in carrying out agency duties.
  • Possess a valid Michigan driver’s license and have a satisfactory driving record.
  • Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.
  • Supports a philosophy of service delivery that is recovery-based, person centered and culturally competent.
  • Is well-organized and self-directed with the ability to manage time in an efficient manner.
  • Possesses high ethical standards, trustworthiness, and is a person of integrity.
  • Maintains appropriate professional image; able to maintain composure and effectiveness under pressure and in changing conditions; able to accept feedback as it relates to job performance and responsibilities.
  • Possesses good clinical judgment and problem solving with ability to de-escalate crisis situations effectively.
  • Possesses excellent interpersonal, oral, and written communication skills.
  • Adhere to applicable policies and procedures as required by the Department of Community Mental Health, the Mental Health Services Board, and other pertinent regulatory agencies.
