Job Title: RN Care Manager

Duration: 3+ months (Potential to Extend)

Location: Fully Remote (must have a CA RN license and work PST hours)

Shift: 8 AM - 5 PM, Monday to Friday (can start at 7 AM if needed)

Day-to-Day Responsibilities

  • Work from a private space due to sensitive information.
  • Comfortable making outbound phone calls from home.
  • Complete 3-4 weeks of training, then work with Senior Case Managers.
  • Handle 5-10 referrals daily, working with Medicare/Medicaid cases.
  • Manage a case load of 75 active cases, ramping up gradually (cases last 90 days).
  • Spend at least 4 hours daily on outbound calls to members.
  • Data entry and tracking using Excel and True Care medical records system.
  • Navigate multiple systems (pharmacy, eligibility, workforce management).
  • Develop, assess, and adjust care plans to promote desired outcomes.
  • Assess members' health status, resource utilization, and treatment plans.
  • Coordinate services between PCPs, specialists, and non-medical staff.
  • Provide patient and provider education.
  • Facilitate members’ access to community-based services.
  • Monitor and track referrals to community organizations and medical care.
  • Participate in integrated team care management rounds.
  • Identify and report risk management quality concerns.
  • Enter and maintain clinical information in medical management systems.

Performance Expectations/ Metrics

  • Meet daily referral assignment and outbound call requirements.
  • Maintain accurate and timely data entry and tracking.
  • Effectively manage a growing case load up to 75 active cases.

Relevant Background/Previous Job Titles

  • Experience in clinical nursing, acute care, or community settings.
  • Roles in case management or working with Medicare/Medicaid.
