Description

The Department of Developmental Services (DDS) has an immediate opening for an Active Treatment Specialist.

The Active Treatment Specialist is responsible for the oversight, coordination, monitoring, and continuous review of Active Treatment for individuals with Intellectual Disabilities/Developmental Disabilities that remain in a nursing facility for greater than 180 days without an immediate placement option. These cases require the development of a Resident Integrated Service Plan (RISP), monthly attendance at the nursing facility, and continuous education to nursing facility staff and providers on the implementation of Active Treatment.

Duties and Responsibilities (these duties are a general summary and not all inclusive):

  • Ensure that high quality assessments are conducted to determine baseline needs for individuals identified as needing Active Treatment.
  • Conduct Resident Integrated Services Plan (RISP) meetings for the development and ongoing quarterly review of an integrated plan of treatment.
  • Coordinate and communicate the plan’s goals with the individual and guardian to ensure full participation and involvement.
  • Provide ongoing education and oversight to active treatment providers regarding the development of plan goals and objectives.
  • Provide ongoing education to nursing facilities regarding the requirements and implementation of active treatment across all settings.
  • Provide monthly monitoring and onsite observation regarding the implementation of plan objectives.
  • Review data for progress or need for changes to goals and objectives.
  • Monthly communication with active treatment providers and nursing facility staff.
  • Document detailed, specific, timely monthly notes in DDS database.
  • Gather and provide documents on a quarterly basis as required for DPH/OQE federal onsite active treatment surveys.
  • Participate in and document any meetings or with a nursing facility or active treatment provider.
  • Develop an annual RISP and document all changes.
  • Participate in active treatment trainings with nursing facilities, active treatment providers, DPH and OQE staff as needed.

Preferred Qualifications

  • Be able to represent the Department effectively, positively, and professionally in a variety of settings.
  • Be able to work well independently and reliably.
  • Have knowledge and expertise in developing collaborative relationships with other state agencies and community resources.
  • Have proficient computer skills preferably in Microsoft products including Word, PowerPoint and Excel.

Qualifications

First consideration will be given to those applicants that apply within the first 14 days.

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

Substitutions

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
