    Relocalizing Health begins with an outline for the future of health that is predicated on a health care system that is local, open and independent. The contours of this success have already been proven. Now it is time to expand on these achievements. The book provides an explanation of not only how much money spent on health care is wasted (over $1 trillion), but how it has devastated public health and stolen away resilience from families and communities all across the United States making us much more vulnerable to the impacts of Covid-19. The book details how money better spent on education, primary care, and public health that build up communities is what we deserve given the talent and treasure invested in health.

    Author, Dave Chase, believes it’s time for the community members to take back control, and that change always happens through grassroots movements. In fact, it’s already happening. The book calls community leaders to join the growing movement. For these movements to be successful, there are concrete steps – included in Chase’s books. Put simply, public and private sector employers and unions must take these steps to stop perpetuating the current system. Using case studies of wise health care purchasers who’ve implemented these changes, plus insight from select health care leaders, Chase’s book is an expert guide on how to bring health back to where it should be – rooted in communities.

    Direct primary care (DPC) is an emerging model that has gained some attention in California and nationally in recent years. Sometimes referred to as "retainer practices," DPC practices generally do not accept health insurance, instead serving patients in exchange for a recurring monthly fee — usually $50 to $80 — for a defined set of clinical services.
    This issue brief describes the landscape of DPC practices, which collectively have more than a half million people on their rolls. Five large practices that use the retainer model are highlighted in the issue brief. The patient rosters are estimations:
    Iora Health, with 2,400 patients
    MedLion, with 3,000 patients
    Paladina Health, with 8,000 patients
    Qliance, with 7,200 patients
    White Glove Health, with 40,000 patients via self-insured employers and 450,000 via health plans
    DPC providers help keep costs low by avoiding unnecessary referrals and by referring mainly to specialists willing to offer significant discounts. Despite this advantage, the DPC model may be hampered by low awareness among health plans and primary care physicians, resistance from some insurers, and resistance from competing hospitals and specialists.

    It’s hard to overstate the implications of doctors being held to the Standard of Care. Many argue that the basis of unnecessary and duplicate procedures is the fear of malpractice. Doctors understand that in a malpractice case one of the key items that is analyzed is whether they met the Standard of Care.

    Many pharmaceutical and medical device giants look remarkably similar to the IBM of the late 80′s and early 90′s. For those of us who remember the IBM of that era, this is bad news. Unfortunately, too many pharma and device companies are following the path of the railroad industry early in the 20th century missing out on a huge opportunity. Alternatively, pharma and device companies can look to the path IBM took when its future prospects were grim. Remarkably, IBM demonstrated how it’s possible for a large company to shift from a product-centric culture to a customer and service centered company. The handwriting is on the wall for many pharma and device companies: They will succeed or fail based not on how many drugs/devices they sell, but on how well their offerings improve health outcomes.

    The marketing myopia of the railroad industry is well documented in the world of business yet most organizations makes the same mistake. Railroad businesses assumed they were in the “railroad” business, rather than the “transportation” business. Consequently, they missed out on countless opportunities to pursue growth in the auto industry. In contrast, IBM was able to use their near extinction to reinvent themselves over a 10 year period. The result has put Lou Gerstner in the pantheon of turnaround CEOs: A 10x increase in IBM’s stock value. In contrast, most pharma companies aren’t spending enough time thinking/acting like IBM. Looking at the 10-year stock charts of these organizations you see flat or declining stock prices. It’s quite clear that reinvention has yet to happen for most pharma and device companies.
