About us
Transition Projects provides individuals with the services, resources and tools they need to end their homelessness, secure housing, and maintain that housing. - Each year, we assist more than 10,000 people experiencing homelessness. - On any given day, we help meet the basic needs of more than 700 people through our Day Center. - On any given night, we provide a safe place to sleep for more than 800 people with nowhere else to turn. - In any given year, we help place more than 800 people into affordable housing - and then support them in retaining that housing. Whenever and wherever we can, we advocate for policies and initiatives that affirm the right of all people to a place they can call home. Want to join our team of passionate changemakers? Transition Projects condemns the Supreme Court’s ruling in Grants Pass v. Johnson, which allows localities to arrest and fine people for living outdoors. Lower courts have ruled that this punishment violates the Constitution’s Eighth Amendment. However, today the Supreme Court ruled that people may be punished because they cannot afford a place to live. We are honored to receive a 2023 Community Benefit Initiative Grant from Trillium Community Health Plan. This four year grant will help to remove barriers to housing by providing legal services for our residents. Thank you, Trillium! Our Laurelwood Shelter — a low-barrier shelter in SE Portland serving female-identifying individuals and couples — is celebrating the two-year anniversary of partnership with Outside In's Mobile Medical Clinic! To date, more than 135 residents have been served during over 325 patient visits! The Outside In clinic includes services like, primary care, check-ups and physical exams, diagnosis and treatment of illness and injury, STI Testing, vaccinations, wound care, chronic disease support, reproductive health care and more. There is also language interpretation services when needed. Thank you, Outside In, for providing vital support for our community facing homelessness.

Residents at the Clark Center — a housing-focused residential program for 90 men — were thrilled to have venison sloppy joes, potato and fruit salad, and ice-cream sundaes provided by Comcast and Hunt. Fish. Feed. volunteers. “Comcast’s partnership with Transition Projects underscores our commitment to serving our communities, particularly those most vulnerable,” said Rebecca Brown, Senior Director of Community Impact for Comcast’s Pacific Northwest region. “Hunt. Fish. Feed. is special because it not only provides a meal but also fosters a sense of community and support for those experiencing houselessness.
