Homeward is rearchitecting the delivery of health and care for rural Americans by connecting payers, providers, and community organizations through partnership. By taking full financial accountability for clinical outcomes and the total cost of care across rural counties, Homeward aligns incentives and strengthens local healthcare ecosystems. The Homeward Navigation™ platform uses advanced analytics to connect members to the right care and local resources that address social determinants of health and improve holistic health outcomes. Since many rural communities lack adequate clinical capacity, Homeward employs care teams that supplement local practices and reach people who cannot access care otherwise. As a public benefit corporation and B Corp™ certified company, Homeward’s mission and business model are aligned to address the healthcare, economic, and demographic challenges that make it harder for rural Americans to stay healthy.

    A recent study from the University of Minnesota Rural Health Research Center found that gaps in access to healthcare can exacerbate risks to the well being of older #rural residents caused by social isolation, injuries, or food insecurity. As the U.S. population ages, many Americans age 65 or older are making their homes in rural communities, and the need for integrated – and sustainable – healthcare solutions has become even more critical. Homeward’s approach and partnership with health plans and health systems in rural communities directly addresses these issues, working closely with providers to connect members to appropriate care and local resources. https://lnkd.in/eFJz23U5

    Established by Homeward Chief Administrative Officer and General Counsel Brian Vandenberg, American Medical Association Foundation’s The Vandenberg Health Equity Fund awards scholarships to future physicians committed to advancing a more equitable, inclusive healthcare system and shaping the future of healthcare. Last year, The Vandenberg Health Equity Fund launched the inaugural Lyle L. Vandenberg Rural Health Scholarship in honor of Brian Vandenberg’s late father Lyle Vandenberg, a University of Wisconsin-Madison alumnus and community pharmacist who spent over four decades serving rural Wisconsin residents. The Lyle L. Vandenberg Rural Health Scholarship awards students in the University of Wisconsin-Madison School of Medicine and Public Health, School of Nursing, or School of Pharmacy who demonstrate a commitment to improving health care access and reducing disparities in small towns and rural communities. This year’s Lyle L. Vandenberg Rural Health Scholarship recipient, Madeline Brown, is passionate about family medicine, values building strong relationships with diverse patients, and promoting healthy lifestyles.     This was another fantastic conversation on mentoring with two incredible physicians, CEOs, Founders, entrepreneurs who are Moms first and foremost. In this #InspiringWomen discussion, we touch on the power of mentorship and Home[ward] Grown mentorship model that Jennifer Schneider, M.D., M. S. and Homeward has put in place. Jenny selects and mentors one female entrepreneur a quarter with unstructured meetings. Time bound, still fits with her busy schedule, high impact. For Sipra Laddha, MD and her company LunaJoy Health, this was by far the best mentoring experience she has ever had and each meeting turned into immediate input to building her company.     I am incredibly invested in empowering and elevating fellow women founders and I’ve been fortunate to have crossed paths with many who are redefining healthcare. Sipra Laddha, MD, co-founder of LunaJoy Health, is one of those women – She’s an entrepreneur who was inspired by her personal journey to motherhood and leveraged her experience as a physician and medical training to reimagine how we support women’s mental health. From puberty, to pregnancy and postpartum, to menopause, and every stage in between, LunaJoy Health was founded to help women thrive and make women’s mental healthcare a priority. I had the pleasure of working closely with Sipra as a Home[ward] Grown mentee over the past several months to help her navigate LunaJoy’s significant growth and progress, have frank conversations, and address top of mind questions.     It was a pleasure meeting with U.S. Department of Health and Human Services (HHS) Secretary Becerra to discuss how we can collectively leverage technology to improve access to healthcare in our communities and learn how the federal government is strategically thinking through the roles of AI and cybersecurity to advance healthcare for Americans everywhere.
