About us
Center for Elders’ Independence PACE offers comprehensive community-based medical and social care all in one bundle. We serve East Bay seniors who are over 55 years of age and who who need nursing home care - but we keep them living at home! CEI personal care teams coordinate and integrate whatever care a senior participant needs - home care, activities, transportation or medical care - everything from a hospital stay to an Oakland A's Game with other participants. Though CEI does not provide housing, we coordinate with senior housing, residential care facilities, hospitals and the family caregivers through any circumstances. Services offered: • Primary Care/Nursing Care • Hospital Services • Emergency Services • Nutrition and Hot Meals • Social Services • Home Health Care • Recreational & Social Activities • PACE Day Center • Medications • Transportation • Medical Specialists (cardiologists, podiatrists, etc.) • Rehabilitation Therapies • Dental Care PACE is associated with lower nursing home visits and admissions, fewer hospitalizations, a higher probability of receiving ambulatory care, greater survival, and increased number of days in the community, better health, better quality of life and better functional status. A contingent of Three Valleys Community Foundation Board, Advisors and staff enjoyed a nonprofit site "field trip" to Center for Elders' Independence Tri-Valley, Alternative Care Setting (ACS). CEO Maria Zamora treated us to a guided tour and we learned about the PACE model, a comprehensive medical/social service delivery system utilizing an interdisciplinary team approach to coordinate all needed preventative, primary, acute and long-term care services to older adults who would otherwise reside in nursing facilities. What a beautiful, spacious, warm and welcoming environment for a diverse group of participants! Highlights: the arts and crafts room, a memory care room, a serving kitchen with menus in multiple languages, two patios, a library and tech/computer learning center, and an expansive rehab gym complete with a mock-up apartment for on-site occupational therapy. #CEI #3VCF #communityoutreach #givegrantgrow #nonprofits #trivalley #givewhereyoulive #coordinatedcare

Senior participants had a fantastic outing at the Oakland Coliseum, cheering on the hometown favorite, the Oakland A's. This activity was facilitated by CEI, which provided transportation and support staff to ensure an enjoyable experience for all attendees. The seniors relished the excitement of the game, the camaraderie, and the vibrant atmosphere, making it a memorable day for everyone involved. Seniors from CEI Tri Valley and PACE Concord gathered in Danville for the second round of the CEI Summer Participant Picnics. With an Olympics theme, the day was filled with engaging games, lively dance performances by participants, and delicious food. Everyone had a fantastic time! Spending time outdoors and socializing is crucial for the health and well-being of seniors. It's picnic season at CEI! PACE Berkeley and PACE Downtown Oakland staff welcomed many participants to the Miller Knox Park in Richmond for a day of games, food and laughter. Staff enjoyed being outdoors and playing games with participants Olympic style! It was a beautiful day at the park with just the right amount of sunshine and breeze. Celebrating Juneteenth with heart and soul! Seniors at San Leandro kicked off festivities early with an enlightening talk on Juneteenth's history, followed by a dance party to live music by Ron Matthews. The song repertoire honored black musician's legacies. Meanwhile, Downtown Oakland and East Oakland seniors were lead in a day of learning and reflection. At a spiritual group meeting, the seniors reflected on a reading of the Maya Angelou poem, "Still, I Rise" and lyrics to the freedom anthem, "A Change is Gonna Come" by Sam Cooke. #JuneteenthCelebration #CommunitySpirit #CEIcares #CEISeniors
