Lets talk rotator cuff repair augmentation for small degenerative tear patterns. First of all is it a "tear" or is it a biologic failure of the rotator cuff tendon bone interface. I would argue the vast majority of rotator cuff pathology is not an acute tear but rather an inability for the body to create a reparative response to a mechanical overload over time. If that is case why would we expect a different long term outcome if we simply put the tendon back in the same place without changing the biology that allowed the failure in the first place. Small tears need biology. What biology are you using in addition to your mechanical fixation? The argument most used for not augmenting small tear patterns is the cost of the implant. I would ask how much does it cost for a revision rotator cuff repair? This patient is 6 weeks out from his workers compensation rotator cuff repair with an Anika Integrity implant. He has near full range of motion, minimal pain, and is back to work light duty. This workers compensation company is very pleased they spent the "extra" money for this patients repair. Yes this is one case, however, experience based evidence is the start of evidence based medicine.
