De Quervain’s Disease is the compression of the tendon sheath of one of the two tendons to the thumb at wrist region.
The tendon sheath is a protective cover for the tendon which provides protection for the tendon as it travels across the radius bone especially at the joints.
When the tendon gets caught in the narrowed sheath, the finger becomes locked in the narrowed tendon sheath resulting in pain and swelling
Women are affected more than men.
It is also known as the washer woman's sprain
Local causes are:
1. Trauma of the tendon sheath of the thumb from too much stress on their thumb from carrying their newborn child especially first time parents
2. Pressure on the tendon sheath from weight of the newborn's head on the wrist while feeding the baby
The onset is usually spontaneous with gradual increasing pains and tightness of the wrist tendons which causes the patient to seek treatment:
1. Swelling and pain of the thumb side of the wrist
2. Unable to open door, bottle caps or wring clothes
Tendon nodules are present in flexor tendon of the thumb at wrist level
Diagnosis is confirmed with CAT scan but may not be needed.
Treatment is by:
1. Rest and splinting of affected thumb and wrist
2. Wrist exercise usually good.
3. Non-steroidal anti-inflammatory drugs for inflammation and pain
4. Corticosteroid injection to reduce inflammation under the narrowed sheath
5. Surgery as last resort to release the tendon by cutting the top part of the narrowed inflamed sheath

He then wrote an autobiography account of his journey as a medical student to family doctor on his other blog http://afamilydoctorstale.blogspot.comThis autobiography account “A Family Doctor’s Tale” was combined with his early “A Simple Guide to Medical Disorders” into a new Wordpress Blog “A Family Doctor’s Tale” on http://ken-med.com.From which many free articles from the blog was taken and put together into 1000 eBooks.He apologized for typos and spelling mistakes in his earlier books.He will endeavor to improve the writing in futures.Some people have complained that the simple guides are too simple.For their information they are made simple in order to educate the patients.The later books go into more details of medical disorders.He has published 1000 eBooks on various subjects on health, 1 autobiography of his medical journey, another on the autobiography of a Cancer survivor, 2 children stories and one how to study for his nephew and grand-daughter.The purpose of these simple guides is to educate patient on health disorders and not meant as textbooks.He does not do any night duty since 2000 ever since Dr Tan had his second stroke.His clinic is now relocated to the Buona Vista Community Centre.The 2 units of his original clinic are being demolished to make way for a new Shopping Mall.He is now doing some blogging and internet surfing (bulletin boards since the 1980's) startingwith the Apple computer and going to PC.The entire PC is upgraded by himself from XT to the present Pentium duo core.The present Intel i7 CPU is out of reach at the moment because the CPU is still expensive.He is also into DIY changing his own toilet cistern and other electric appliance.His hunger for knowledge has not abated and he is a lifelong learner.The children have all grown up and there are 2 grandchildren who are even more technically advanced than the grandfather where mobile phones are concerned.This book is taken from some of the many articles in his blog (now with 740 posts) A Family Doctor’s Tale.Dr Kee is the author of:"A Family Doctor's Tale""Life Lessons Learned From The Study And Practice Of Medicine""Case Notes From A Family Doctor"

    Ode to De Quarvain’s Disease

    Dequervian tendonitis is compression of the tendon

    Of the thumb from a narrowed tendon sheath at the wrist region

    When the tendon gets caught in the tight tendon sheath

    The finger can become locked and is unable to move with ease

    The tendon becomes inflamed with swelling and pain

    Women are affected more than men

    Often it is due to the baby head lying on the wrist

    While milk is being fed to the baby like this

    Do not use the thumbs to do repetitive movements

    Avoid a direct blow to the area of the first dorsal compartment

    There will cause the tendon’s tunnel sheath to be compressed

    As a result the sheath tunnel will collapse and become depressed

    The treatment is to try exercising the fingers in warm water

    And stretch the tendon through the tunnel’s depressed quarter

    If that do not work you may need a shot of cortisone into the tendon sheath

    Or Surgery to cut the narrowed tendon sheath for the tendon’s relief

    A Healthy Lifestyle

    1. Take a well Balanced Diet

    2. Treatment:

    a. Rest and splinting of affected thumb and wrist

    b. Wrist exercise usually good.

    c. Non-steroidal anti-inflammatory drugs for inflammation and pain

    d. Corticosteroid injection to reduce inflammation under the narrowed sheath

    e. Surgery as last resort to release the tendon by cutting the top part of the narrowed inflamed sheath
