Dandruff[1] (Latin: Pityriasis simplex capillitii[1]) is the shedding of dead skin cells from the scalp. Dandruff is sometimes caused by frequent exposure to extreme heat and cold and is not to be confused with a simple dry scalp. As skin cells die a small amount of flaking is normal; about 487,000 cells/cm2 get released normally after detergent treatment.[2] Some people, however, experience an unusually large amount of flaking either chronically or as a result of certain triggers, up to 800,000 cells/cm2, which can also be accompanied by redness and irritation. Most cases of dandruff can be easily treated with specialized shampoos.

Dandruff is a common scalp disorder affecting almost half of the population at the post-pubertal age and of any sex and ethnicity. It often causes itching. It has been well established that keratinocytes play a key role in the expression and generation of immunological reactions during dandruff formation. The severity of dandruff may fluctuate with season as it often worsens in winter.[2]

Those affected by dandruff find that it can cause social or self-esteem problems, begging treatment for both physiological and psychological reasons.[3]

Causes [link]

As the epidermal layer continually replaces itself, cells are pushed outward where they eventually die and flake off. For most individuals, these flakes of skin are too small to be visible. However, certain conditions cause cell turnover to be unusually rapid, especially in the scalp. For people with dandruff, skin cells may mature and be shed in 2–7 days, as opposed to around a month in people without dandruff. The result is that dead skin cells are shed in large, oily clumps, which appear as white or grayish patches on the scalp, skin and clothes.

Dandruff has been shown to be the result of three required factors:[4]

  1. Skin oil commonly referred to as sebum or sebaceous secretions[5]
  2. The metabolic by-products of skin micro-organisms (most specifically Malassezia yeasts)[6][7][8][9][10]
  3. Individual susceptibility

Older literature cites the fungus Malassezia furfur (previously known as Pityrosporum ovale) as the cause of dandruff. While this species does occur naturally on the skin surface of both healthy people and those with dandruff, in 2007 it was discovered that the responsible agent is a scalp specific fungus, Malassezia globosa,[11] that metabolizes triglycerides present in sebum by the expression of lipase, resulting in a lipid byproduct oleic acid (OA). During dandruff, the levels of Malassezia increase by 1.5 to 2 times its normal level.[2] Penetration by OA of the top layer of the epidermis, the stratum corneum, results in an inflammatory response in susceptible persons which disturbs homeostasis and results in erratic cleavage of stratum corneum cells.[8]

Dandruff can also be a manifestation of an allergic reaction to chemicals in hair gels, sprays, and shampoos, hair oils, or sometimes even dandruff medications like ketoconazole.[citation needed]

There is some evidence that excessive perspiration and climate have significant roles in the pathogenesis of dandruff.[citation needed]

Composition [link]

Dandruff scale is a cluster of corneocytes, which have retained a large degree of cohesion with one another and detach as such from the surface of the stratum corneum. The size and abundance of scales are heterogeneous from one site to another and over time. Parakeratotic cells often make up part of dandruff. Their numbers are related to the severity of the clinical manifestations, which may also be influenced by seborrhea.[2]

Treatment [link]

Shampoos use a combination of ingredients to control dandruff. The pathogenesis of dandruff involves hyperproliferation of keratinocytes, resulting in deregulation of keratinization. The corneocytes clump together, manifesting as large flakes of skin. Essentially, keratolytic agents such as salicylic acid and sulphur loosen the attachments between the corneocytes and allow them to get swiped off.

Zinc pyrithione (ZPT) heals the scalp by normalizing the epithelial keratinization or sebum production or both. Some studies have shown a significant reduction in the number of yeasts after use of ZPT, which is an antifungal and antibacterial agent.[12] A study by Warner et al.[13] demonstrates a dramatic reduction of structural abnormalities found in dandruff with the use of ZPT; the population abundance of Malassezia decreases, parakeratosis gets eliminated and corneocytes lipid inclusions are diminished.[2]

Egg oil [link]

In Indian,[14] Japanese, Unani (Roghan Baiza Murgh)[15] and Chinese[16] traditional medicine, egg oil was traditionally used as a treatment for dandruff.

Steroids [link]

The parakeratotic properties of topical corticosteroids depend on the structure of the agent, the vehicle and the skin onto which it is used. Corticosteroids work via their anti-inflammatory and antiproliferative effects.[17]

Selenium sulfide [link]

It is believed that selenium sulfide controls dandruff via its anti Malassezia effect rather than by its antiproliferative effect, although it has an effect in reducing cell turnover. It has anti-seborrheic properties as well as cytostatic effect on cells of the epidermal and follicular epithelium. The excessive oiliness after use of this agent has been reported in many patients as adverse drug effect.

Imidazole antifungal agents [link]

Imidazole topical antifungals such as ketoconazole act by blocking the biosynthesis of ergosterol, the primary sterol derivative of the fungal cell membrane. Changes in membrane permeability caused by ergosterol depletion are incompatible with fungal growth and survival.[18]

Ketoconazole is a broad spectrum, antimycotic agent that is active against both Candida and M. furfur . Of all the imidazoles, ketoconazole has become the leading contender among treatment options because of its effectiveness in treating seborrheic dermatitis as well.[2]

Hydroxypyridones [link]

In contrast to the imidazole antifungals, the hydroxypyridones do not affect sterol biosynthesis; instead they interfere with the active transport of essential macromolecule precursor, cell membrane integrity and the respiratory process of cells. Ciclopirox is widely used as an anti-dandruff agent in most preparations.[19]

Seborrhoeic dermatitis [link]

Specific diagnosis and treatment of dandruff may be difficult as its spectrum blurs with those of seborrhoeic dermatitis and some other scaly. In seborrhoeic dermatitis redness and itching frequently occurs around the folds of the nose and eyebrow areas, not just the scalp. Dry, thick, well-defined lesions consisting of large, silvery scales may be traced to the less common affliction of the scalp psoriasis.

Inflammation and extension of scaling outside the scalp exclude the diagnosis of dandruff from seborrhoeic dermatitis.[5] However, many reports suggest a clear link between the two clinical entities - the mildest form of the clinical presentation of seborrhoeic dermatitis as dandruff, where the inflammation is minimal and remain subclinical.[20][21]

Seasonal changes, stress, and immuno-suppression seem to affect seborrheic dermatitis.[2]

Limbo

In the theology of the Catholic Church, Limbo (Latin limbus, edge or boundary, referring to the "edge" of Hell) is a speculative idea about the afterlife condition of those who die in original sin without being assigned to the Hell of the Damned. Medieval theologians of western Europe described the underworld ("hell", "hades", "infernum") as divided into four distinct parts: Hell of the Damned,Purgatory, Limbo of the Fathers or Patriarchs, and Limbo of the Infants. However, Limbo of the Infants is not an official doctrine of the Catholic Church.

Limbo of the Patriarchs

The "Limbo of the Patriarchs" or "Limbo of the Fathers" (Latin limbus patrum) is seen as the temporary state of those who, despite the sins they may have committed, died in the friendship of God but could not enter Heaven until redemption by Jesus Christ made it possible. The term "Limbo of the Fathers" was a medieval name for the part of the underworld (Hades) where the patriarchs of the Old Testament were believed to be kept until Christ's soul descended into it by his death through crucifixion and freed them (see Harrowing of Hell). The Catechism of the Catholic Church describes Christ's descent into hell as meaning primarily that "the crucified one sojourned in the realm of the dead prior to his resurrection. This was the first meaning given in the apostolic preaching to Christ's descent into hell: that Jesus, like all men, experienced death and in his soul joined the others in the realm of the dead." It adds: "But he descended there as Saviour, proclaiming the Good News to the spirits imprisoned there." It does not use the word "Limbo".

Limbo (Bryan Ferry song)

"Limbo" is a song by Bryan Ferry, the former lead vocalist for Roxy Music. It was released as the third and final single from his seventh album Bête Noire in 1988, being Ferry's twenty-seventh single. The song failed to enjoy as much success as the two previous singles from the album. The song peaked at number 86 on the UK Singles Chart. It also appears in the ill-fated film Big Time, starring Paul Guilfoyle.

The song, alike all the other single's released from the album ("The Right Stuff", and "Kiss and Tell"), it features The Smiths' lead guitarist, Johnny Marr.

The promotional video for the song was directed by style and fashion guru Michael Roberts, a longtime friend of Ferry's.

Compilation appearances

As well as the song's album and single releases, it has been featured on various compilation albums released by Ferry. Including "The Platinum Collection", "Collection", and "The Best Of Bryan Ferry".

Limbo (DC Comics)

Limbo refers to a fictional location in books published by DC Comics. Limbo first appeared in Ambush Bug (vol. 1) #3 (August 1985), and was created by Keith Giffen.

History

In Ambush Bug (vol. 1) #3, Jonni DC mentions removing Wonder Tot from DC continuity having "dumped her in Limbo." Ambush Bug later returns to Limbo in Son of Ambush Bug #6.

Animal Man

In Animal Man #25 (July 1990), Grant Morrison reintroduces Limbo, a dimension inhabited by old characters seemingly abandoned or forgotten by their publishers. The comic depicted such characters as the Inferior Five, Mr. Freeze, and the Gay Ghost (who expressed a desire not to be revived). This comic was published in the wake of DC's Crisis on Infinite Earths, in which many historic comics were removed from continuity. This "comic book limbo" is a metafiction, based on the notion that any character who has not been published recently can be said to reside in "comic book limbo".
