Our Transition Service helps individuals who qualify to transition into adult social care. The team conducts a Needs Assessment to determine the appropriate adult care package. This plan is implemented before the individual turns 18, ensuring all necessary support is ready upon adulthood. There is a lack of a clear pathway for young people stuck in limbo from the ages of 14 to 18. They are trapped in safety plans and assessment outcomes that they have outgrown and cannot receive a new care package before turning 18.  But we have a solution. Thanks to our managerial capacity and close relationship with our sister company, Advanced CCA, we bridge the gap between children’s and adult services and support individuals throughout the process. Our robust support system promotes efficiency and communication to deliver fast and proactive care through quality assessments. #AdultSocialCare #Transition #Safeguarding
