M&A and turnaround of healthcare facilities!   Took the Masterclass on 28 September.   India is the fastest growing major economy. It is the fifth largest economy and the third largest in terms of PPP. Healthcare industry is one of the biggest employers employing ~7.5 million people. Current size of the healthcare industry is $372 billion and is expected to increase to $638 billion by 2025, growing at a CAGR of 16-17%. India still spends less than 5% of its GDP on healthcare, as against the OECD countries 8-10% and the US a whopping 20%.   Most legacy providers are consolidating aggressively and almost 30000 crores has been invested over the last 4 years with some mega deals have also been signed, powered by PE firms. Part of this investment could be used to extend delivery to Tier2/3 towns wherein the care delivery remains abysmal.   Integration of the newly acquired healthcare facilities (brownfield projects) becomes paramount for the providers/ PE firms to cerate value for their stakeholders. It entails design and execution of a prudent comprehensive business strategy.   Greenfield healthcare projects present a unique challenge to operationalize and turn profitable since all expenses are front loaded. In the absence of comprehensive strategy, turnaround is well-nigh impossible.   I’ve brought out key highlights of the same during my masterclass. FACT Hai… Gyan Nahi… Shift in Indian Health Story – and Long term Government Initiatives. In recent years, the Indian government has introduced significant reforms to strengthen the healthcare sector. A key initiative, Ayushman Bharat, aims to enhance the health system comprehensively from primary to tertiary care, marking a shift from vertical health programs. Its first component focuses on building a next-generation primary healthcare network through Health and Wellness Centres (HWCs) to promote good health and enable early disease detection, particularly for non-communicable diseases (NCDs), which are rising in India. These HWCs prioritize disease prevention and extensive screening, offering sustainable solutions to reduce disease burden and minimize out-of-pocket healthcare expenses, which currently make up more than 60% of health costs. Additionally, the National Digital Health Mission (NDHM) seeks to digitize health records and create an efficient system for managing and exchanging health data. This initiative will help develop registries of healthcare providers, laboratories, and pharmacies while supporting clinical decision-making and enabling services like telemedicine. By improving transparency and efficiency, the NDHM is expected to unlock over USD 200 billion in economic value over the next decade, providing patients with better access to healthcare information, provider credentials, and pricing. The Eldercare sector is ready for disruption but there are several challenges that we, as entrepreneurs, should turn into opportunities. The current Taxation regime for Homecare and Eldercare makes the services prohibitively expensive. It hinders the consolidation and formation of an organized sector in this area. A country like India with a growing elderly population needs to bring healthcare to home. The government should not look at this sector as a source of revenue for the government. A lot still needs to be done in Remote Health Monitoring and Management and Emergency Response Management to make home health care a compelling proposition for our elders and loved ones. We need to improve training and skill development infrastructure and build a robust supply chain for efficient home health care administration. Here’s some great news for health tech / age tech! My main man Anurag Ram Chandran and his co-conspirator Ayush Agrawal have launched Kubo Care Private Ltd : a technology platform to assist senior living. I've seen their PoCs, and they are doing some really exciting work. Big props to Antler India for getting this talent together and giving them an ecosystem to conceive and then start executing this idea! More than the PoC, I've seen the guys upfront and I can tell you that they really want to solve for better living for our senior citizens. With more and more intra and inter national migration of the newer generation, we need technology to help us take care of fastest growing population in India today : our senior citizens. Healthy, safe, happy, respectable and comfortable aging is not just a social responsibility to maintain the fabric of society, but also a massive business opportunity. The middle class 60+ population is also more independent than ever. They don’t want to travel to the US and spend time in cold places where they are always dependent on someone to drive them around. They want their activities, community, shopping and routines (which are often better than those of much younger people). Technology to enable this is a massive opportunity. My friends at Ubiqare Health have also been working on this opportunity : remote monitoring technology for healthy to palliative usecases. The beauty of building a company is that you can create something that never existed. Here’s hoping that Kubo et al can create something brilliant, that our elders will be proud of and delighted to use 😁

I do wonder if the greater un-met need for healthcare in markets with significant resource constrained healthcare systems will drive the adoption of digital healthcare faster than in countries with more established healthcare systems. What I mean by this, and noting I'm making a broad strokes comparison, will the more urgent need to get access to healthcare in markets like southeast Asia, India etc make consumers and physicians more open to using innovative digital technologies compared to the US, Europe, Australia etc where the un-met healthcare need gap is less urgent? The reason I post this is I keep seeing companies in India, particularly pharma companies, launching and testing new healthcare services centered around digital therapeutics and other digital health products (see my previous post about Rambaxy Pharma). As far as I can see western pharma companies are avoiding the digital therapeutics space and I do not see any of them going in the direction of creating digital health enabled patient services. To be fair a reason for this is a big difference in commercial models and focus. Western pharma companies have moved towards high-priced specialty drug portfolios, which is different to the generic drug focus / heritage of companies coming out of other countries like India. The value created by working directly with patients might not be financial attractive to western pharma companies, and the need to directly support healthcare delivery may be less urgent and less viable in western markets. So are we see a new wave of innovation in the healthcare space in countries where there are significant gaps in healthcare availability, which in turn is driving the development, testing and use of new digital health / digital therapeutics products and services? This certainly seems to be the case when looking at the deployment of digital health enabled care services and products in the Indian market as highlight by the article below about Lupin's digital health commercial activities. #digitalhealth #digitaltheapeutics
