Use unified opcode table for more instructions

Ports opcode 19 and 31 to the unified opcode table. Also uses templates
to create specialized versions of the branch instruction implementations
to avoid some branching depending on which flags are set.

Takes the benchmark from ~430 to ~439 MiB/s.
