Conclusion
La coagulopathie du choc hémorragique traumatique est un phénomène
complexe et multifactoriel. Des progrès ont été réalisés concernant la connais-
sance des mécanismes physiopathologiques impliqués, les outils diagnostiques
disponibles et l’éventail des possibilités thérapeutiques. Cependant, la plupart
des connaissances actuelles reposent sur des études observationnelles ou
précliniques. Des essais cliniques de méthodologie plus robuste sont nécessaires
afin de déterminer les bénéfices réels des nouveaux moyens de monitorage
(TEG®/ROTEM®) et l’efficacité des différentes stratégies thérapeutiques (PFC
et/ou concentrés de facteurs de coagulation).
