OIG does not attempt to identify persons contacting the Hotline, and no identifying information is shared when you submit an e-mail. You may provide your name, address, or phone number, if you wish.
Indications of the following: Management/supervisory retaliation; contract and procurement irregularities; conflicts of interest; theft and misuse of property; travel fraud; misconduct; time and attendance abuse; misuse of Government credit card; abuse of authority; program mismanagement; misuse of information technology resources; substantial and specific danger to the public health and safety; computer crimes; and other violations of Federal laws and regulations.
A brief, accurate statement of facts believed to provide evidence of wrongdoing; names, addresses, and office locations of pertinent individuals and organizations; dates and times when the suspected wrongdoing took place or is expected to occur; how you became aware of the information; and memoranda, contracts, invoices, or other related documents.
