CRITICAL THINKING EXERCISE:
1. A 70 year old woman is referred to the allergy and immunology clinic for
evaluation of her immune status in addition to the eight medication she routinely
takes, she has received several courses of antibiotics in the previous months for
recurrent infections. What diagnostic test would you expect to be ordered? What is
the rationale for these? What further assessment data would you want to obtain
from this patient?
Answer: Blood test.
Rationale: Blood typing is done prior to a blood transfusion or when classifying a
person’s blood for donation. Blood typing is a fast and easy way to ensure that
you receive the right kind of blood during surgery or after an injury. If you’re given
incompatible blood, it can lead to blood clumping, or agglutination, which can be
fatal.
Further Assessment: Assess the patient for family history, lifestyle and thorough
physical examination provide useful data for the diagnosis and management of
allergic disorders.
2. A 36 year old woman is hospitalized for a heart transplant and
immunosuppressant medication are prescribed. Described how her altered
immune functions would affect the care that you provide. Develop an evidence-
based teaching plan for the patient and her family before hospitalized discharge.
Discuss the criteria used to assess the strength of the evidence for your teaching
plan.
3. A 24 year old woman is diagnosed with systemic lupus erythematous (SLE), and
corticosteroids are prescribed. What nursing observations and assessments are
indicated? Identify patients teaching that is appropriate for the new diagnosis and
presentation of steroids.
Answer: The appropriate nursing teaching that should be given to the patient, is
that systemic lupus erythematous (SLE), is a chronic autoimmune disease that
causes a systemic inflammatory response in various parts of the body. The
cause of the said disease is still unknown, and according to studies that SLE is
considered to be incurable. Treatment for this disease, focus on the alleviation of
symptoms and to only promote comfort for the patient. The treatment for this
disease varies from the severity of its symptoms. Steroidal drugs are commonly
given to SLE patients, for its immunoregulatory properties for they suppress the
activity of the immune system.
Health Teaching:
- Educate the patient regarding to the drugs side effects such as blurred vision,
water retention, increased appetite and weight gain, and also stomach
irritation.
- Assess the patient for presence of the following the condition such as HIV or
AIDS, Tuberculosis, stomach or intestinal problem, diabetes, glaucoma, high
blood pressure and any form of infection.
- Observe the kind of food that the patient consume, during the treatment,
some foods can have a negative interaction towards the drug.
- Tell the Importance of right dosage of the medication
- Monitor the blood pressure of the patient as well as his/ her bone density and
ask the patient to limit tobacco and alcohol consumption.
- Tell patient the side effect/adverse effect of the medication if over consume
- Advice patient to notify prescriber if any adverse effect happened
