Refactor pgbench code for XC and add documentation
There is an additional mode in pgbench dedicated to Postgres-XC
to help testing easily the write-scalability of the system tested.
By using option -k at initialization, it is possible to force the
tables to be hash-distributed by column bid.
When launching a test, once again using -k allows to enforce write
queries to use an additional condition on bid column to reduce the
node selectivity inside transaction into a single node. This helps
testind the write-scalability of the system by dividing write load
based on a unique key, and reduces the effects of locks due to the
update of pgbench_accounts.
At the same time some old bugs are fixed.

Fix snapshot management on slave nodes and Datanodes accessed directly by applications
The following things are changed:
- Make a slave node session acquire its snapshot directly from WAL
like PostgreSQL does. We do not try to get a global snapshot from GTM
- Allow a Datanode to get a snapshot directly from GTM when accessed directly
by an external application
- Block run of queries interacting with remote nodes on a slave nodes.
In this case it is necessary to obtain a transaction ID but a node in
recovery cannot do that.
in consequence, the following issues are fixed:
- Allow read queries to run on Coordinator slaves by allowing getting a
snaphot. A slave Coordinator was complaining about the impossibility to get
a transaction ID when fetching a snapshot for all types of queries. This allows
to check if the slave Coordinator is still alive and to monitor its statistics
as long as the query does not involve data located on remote Datanodes.
- Suppression of the annoying WARNING "Cannot get a global snapshot from GTM"
when connecting to a Datanode directly with a psql client. Now in this case
a snapshot is taken directly from GTM. Processes like autovacuum did that already.
- Make slave nodes getting there snapshots directly from WAL and not from GTM.
This is also backpatched to 1.0.

Refactor pgxc_monitor: help option bug and overall review
pgxc_monitor was not printing help even if requested by "--help"
or "-?". This has been reported by Nikhil Sontakke but I did not
use his patch.
Some whitespaces and spelling mistakes in code are corrected.
Documentation had also some mistakes but it is fixed now.
The creation of the psql command used to monitor nodes is made
more readable and now facilitates future extensions.

Refactoring of pg_regress.c due to bugs with O2 compilation
When code was being compiled with O2, initialization and start-up
of GTM was done twice, creating errors in log files.
In consequence, functions related to start and initialization of
PGXC nodes and GTM are unified to clarify code and additional checks
are added to verify that nodes have been correctly started.

Fix for bug 3556031: pgadmin3 connection error
pgadmin3 parses the output of "SELECT version();" to determine
the server version it is connecting to. For Postgres-XC, this
output result was changed and completed with Postgres-XC and
PostgreSQL version. pgadmin thought that the version of server
was the one of XC, meaning 1.0 for 1.0 stable and 1.1 for master
branch. In consequence to that, SQL queries with very old catalog
were run to server, and of course errors were reported to client,
and pgadmin cannot work correctly.
This is solved with changing back version() to its former output.
An additional function pgxc_function() is added to provide a way
for application to inform about Postgres-XC version and the PostgreSQL
version it is based on.
Change is backpatched to all supported versions.

Fix for bug 3556031: pgadmin3 connection error
pgadmin3 parses the output of "SELECT version();" to determine
the server version it is connecting to. For Postgres-XC, this
output result was changed and completed with Postgres-XC and
PostgreSQL version. pgadmin thought that the version of server
was the one of XC, meaning 1.0 for 1.0 stable and 1.1 for master
branch. In consequence to that, SQL queries with very old catalog
were run to server, and of course errors were reported to client,
and pgadmin cannot work correctly.
This is solved with changing back version() to its former output.
An additional function pgxc_function() is added to provide a way
for application to inform about Postgres-XC version and the PostgreSQL
version it is based on.
Change is backpatched to all supported versions.

For pseudotypes we do not have corresponding _in() functions defined. Since the
results from the datanode are sent to coordinator in text form, we need to use
_in() functions to convert those into internal representation. Hence results
which have pseudotype data as their part can not be accepted by coordinator.
Hence targetlist expressions which are of pseudotype are not shippable.

Stop write operations on Datanodes when not accessed from Coordinator
XactReadOnly is completed by an XC-specific condition at transaction startup
insuring that Datanode backends accessed directly from external application
are read-only. This protects Datanodes from extern write operations, as they
may compromise the data consistency in cluster.
A couple of whitespaces have been cleaned at the same time.
