Installation

This is a web application that will run in Apache with PHP and MySQL is required to manage the data.

Web Site

Download the tarball and unpack it and upload it to a web folder on your web server. "chown" to the user for Apache so phpFormGenerator can create and edit files/folders. Once you have done this, you need to create your database and then link it to your DB.

Database

For this you will need to choose a database name, user, and password. We will assume phpformgenerator for the DB name and the User name and the password. Please change them to something more secure for a production installation.

Create the database

Modify the database link file: config.inc.php to place your credentials in it. Be sure you test them as the program will not run without permissions.

Add the tables to the newly created database. This information was culled from the apparently dead phpformgen project and it still a work in progress. Please note any irregularities and we'll bring this up to date a bit at a time. (And then we'll put it in the download package as an executable .sql as well!)

Be sure you modify the "USE" to your actual db name.

Hi, i've tried using your phpformgen i've followed all your instruction to install the package. i've used Wamp and PhpMyAdmin to create database. Also edited config.inc.php with related database credentials but i'm getting error when trying to create a form. The error is "Program Malfunction, Please Start Over" i've attacahed screen shots too. please look it it.
It would be beautiful if it works as per your video.
