Got glue working, as demonstrated by frac

First pass with a real TeX box model. Lots of things broken -- just
want to mark this spot in the revision history.

Make draw_bitmap inner loop faster and prevent segfaults if x and y
are negative.

Clean up logging. Remove dead function.

Make \under and \over behave as in TeX. (i.e. it's {x \over y} not \over{x}{y})

Oops. Dotless i was already in there as \imath

Make Cairo and Gdk backends work with new mathtext refactoring.

Support dotless i and j

Fix spacing when going from non-math to math.

Got SVG and PS (afm-only) support working. Still kind of tempermental.
