code cleanup:
* rename CRegStdWORD to CRegStdDWORD
* replace the stdstring defing with a typedef for tstring
* get rid of the wide_string typedef, use std::wstring instead

Pass the passphrase entered in the url-to-monitor dialog to the svn lib when asked for a passphrase for a cert-file.

Use a memory mapped file for loading the url infos.
