Changed installer project and script files from MPL v1.1 to MPL v2.0

Added svn:mime-type, svn:eol-style and Date and Revision svn:keywords properties.

Added MPL v2.0 to help project, index and TOC files. These files were previously unlicensed.

Added missing DOCTYPE

Changed various help files from MPL v1.1 to MPL v2.0

Removed Rev and Date svn:keywords properties from file to prevent it from updating $Rev$ and $Date$ placeholder containing in the boilerplate code.

Added file containing boilerplate comments used for adding MPL 2.0 to source files. Contains comments in styles used for various languages.

Changed header comments to conform to format used by rest of file format files.

Fixed bug #3561713 (http://bit.ly/QEYRFx): Category overview sometimes doesn't start per preferences.

Main form was indirectly changing the tab in the overview pane when starting up with any except Categories tab (tab index 0) selected. This was saving state of tab 0 before it was assigned any tree nodes, meaning that it was displayed expanded when next selected because of lack of state info. Fix meant providing any Initialise method for the overview pane that selects the required tab without saving the state of tab 0. Main form calls this via new Initialise method of TMainDisplayMgr.

*** This fix is very similar to that applied to the trunk at r2304 to fix same bug in v4 code base (that time reported as bug #3561047).
