Make use uf the Databasetable Builder

Refactoring to use the builder for the database fields instad of the constructor

Removed all usages of the default constructor of DatabaseTableIndex and replaced it with builder use

Rename the class

Refactered the constant interfaces into final classes

Do not make the tablename upper case. If the user input is wrongly spelled it is an input error and should not be corrected by som obscure internal magic. This would also disallow lowercase table names.
